The barriers have been lowered for more residents to receive high-quality care in the local region.
The Hastings Prince Edward Ontario Health Team has successfully reduced the Health Care Connect wait list for residents registered before January 1, 2025, from 11,105 unattached patients to zero.
Through a collective effort involving the HPE OHT, Primary Care Network, and Ontario Health atHome, the wait list was thoroughly reviewed and updated.
Following this validation process, which determined the actual number of unattached patients in the region, there were 5,147 residents identified as still needing a family physician or nurse practitioner and were successfully connected.
The team was spearheaded by Barinder Gill, PCN Executive Co-Lead and Clinical Co-Leads Dr. Sarah Le Blanc and Dr. Robert Pincock.
With this accomplishment, the HPE OHT and Primary Care Network are now focused on connecting the remaining residentsĀ on the provincial wait list to a family physician or nurse practitioner.
As of Tuesday, there are 3,226 unattached patients remaining on the Health Care Connect wait list who registered after January 1, 2025, with many more potentially being underrepresented.

All that is required is a valid Ontario Health Card and a current mailing address associated with the health card.
Residents already on the wait list should ensure their contact information is current so they can be reached when a family physician or nurse practitioner becomes available.
